Abstract

This case report describes a rare presentation of a large periapical lesion, suspected to be a radicular cyst, associated with a previously treated maxillary central incisor. The lesion impinged upon the incisive foramen, and the patient presented with persistent pain and worsening migraine episodes unresponsive to pharmacological management. Surgical management of the lesion resulted in significant symptom resolution. This case highlights the need for dental professionals to consider odontogenic sources of chronic facial pain and emphasizes the importance of anatomical relationships in interpreting unusual orofacial symptomatology.
